The admin was logged on and supered at the time. I wouldn't rule out a possible breach though. 3. No 4. No. 5. This is essentially a security removal as I'm unsure whether this was intentional given the recent breaches. If not, the admin was well aware of what they were doing.

I supered games_creator yesterday as he was flagged, he did verify but i'm unsure if this is a breach.

in this case, there was no other IP involved - no breach just buffoonery.
