Holy shit just encrypt passwords instead of hashing them, use TLS properly, don't hire retarded system/database administrators that actually hand the database to social engineers, and then this won't be a problem
Although TLS by itself won't secure against cookie phishing, it's how you use it. The best method being used by both GitHub and Google is TLS_ECDHE_RSA_WITH_AES_128_GCM_SHA256 with 128 bit keys. It would be better if people started using something like TLS_ECDHE_RSA_WITH_AES_256_GCM_SHA512 with 256 bit keys but that's just an example.
